On the day you arrive at Chaophya Hospital , you will be greeted by an Admissions and Registration receptionist. Please have with you:
Your personal identification ( a passport or a driver license )
Medical Records, x-ray films, or prior test results
Your Health Insurance Card
Written letter of authorization from insurance( if you are covered by worker's compensation)
The receptionist will issue you and identification bracelet with your name and Chaophya history number. Please do not take the bracelet off until you have been discharged from the hospital
We encourage you to bring only essential items to the hospital. For your own peace of mind, please do not bring jewelry or large sum of cash with you.
(Please arrive approximately 15 minutes early to insure we have captures all of your information)
Appointment
We encourage you to make your appointments as far in advance as possible.
Cancellation
If you can not keep a schedule appointment, please notify us as soon as possible or at least 24 hour in advance.
